If you are travelling abroad and require vaccinations you will need to fill out the below Travel Risk Assessment Form and bring it down to the surgery and the Nurse will contact within a week to tell you if you need any vaccinations.
Some courses of vaccinations take two or three months to complete so please take this into consideration when planning your journey.
